Agreement and Consent Form – Planetherapy Service Recipients

Planetherapy was founded by volunteers with the goal of connecting individuals in need of emotional support due to the impact of war with therapists or personal coaches willing to assist. While the support and emotional assistance provided during the sessions come from professional therapists, they are not defined as therapy sessions.

To participate in Planetherapy, I confirm, understand, and declare the following:

• I confirm that I am over 18 years old.
• I agree to participate in Planetherapy voluntarily and of my own free will. I acknowledge that I am under no obligation to participate and that I may discontinue my participation at any time.
• I understand that participation in Planetherapy is free of charge and that the initiative is organized voluntarily by the founders, the Planetherapy team, their partners, and/or their representatives (hereinafter: Planetherapy team).
• I confirm, acknowledge, and understand that my participation in Planetherapy is entirely and solely my responsibility. I declare that I will have no claims of any kind against the organizers, initiators of Planetherapy, their partners, or representatives regarding my participation and/or the support I receive.
• I acknowledge that the Planetherapy team does not guarantee the provision of emotional support, finding a therapist, or any specific outcome from the sessions. The team is not responsible for the content of the sessions, their outcomes, or any harm that may arise (if any) from my interaction with the therapist, my relationship with them, or my satisfaction with the support provided (if given).
• I acknowledge that the Planetherapy team is not obligated to find a therapist for me (if one is found) within a specific timeframe or according to any strict schedule.
• I understand that the connection between me and the therapist is made at the sole discretion of the Planetherapy team. The team is not responsible for the match itself or its results. The connection (if made) depends on therapist availability within Planetherapy, and there may be mismatches between my requests and actual availability.
• Planetherapy offers up to six (6) free sessions. Please note that every scheduled session is counted unless canceled at least 24 hours in advance.
• I acknowledge that the Planetherapy team cannot verify or authenticate the credentials provided by the therapists, either in full or in part, and that the team is not responsible for the quality of the sessions.
• I understand that the therapists participating in Planetherapy have varying qualifications, including licensed psychologists, social workers, occupational therapists, art therapists, expressive therapists, coaches, communication specialists, and others. I confirm that it is solely my responsibility to verify the credentials of the therapist I interact with through Planetherapy.
• I acknowledge that to participate in Planetherapy, I will be required to provide personal details, including but not limited to my first name, last name, phone number, gender, and information regarding the personal circumstances requiring support. I understand that I am not obligated to provide any personal information and that any information I choose to share (including in the participation form) is given voluntarily and with my consent.
• I acknowledge that my personal information will be kept confidential and stored securely by the Planetherapy team.
• I consent to my personal information, in whole or in part, being shared with participating therapists, even if no suitable match is ultimately found for me.
• I acknowledge that the Planetherapy organizers, initiators, and team do not act on behalf of the therapists, nor do they represent them. Their role is solely to facilitate the initial connection.
• I acknowledge that any support I receive (if I receive it) through Planetherapy does not substitute for professional medical or psychological treatment if required in my situation.
• I understand that the Planetherapy team reserves the right to discontinue Planetherapy at any time and at their sole discretion.
• If the session is intended for a child under 18 years old, a parent or legal guardian must be present. In the case of divorced parents, written consent from both parents is required.
